Physical Exams

School physicals needed by grade for Illinois students (click the link to download the appropriate form):

Preschool: Illinois Physical

Kindergarten: Illinois Physical , Illinois Dental, and 2016 Illinois Vision Form examinations are required. All kindergarten students will be required to have 2 doses of MMR and 2 doses of Varicella.

Second: Illinois Dental examination is required.

Fifth: No physical is required unless participating in school team sports.

Sixth: Illinois Physical  and Illinois Dental examinations are required. Tdap Booster is required, as well as 2 doses of Varicella and 1 dose of Meningococcal Conjugate Vaccine (MCV4).

Seventh and Eighth: No physical is required unless participating in school team sports.

Physical, dental and eye exam forms are to be turned in by the first day of student attendance.

Sports physicals are required for students participating in sports in grade five through eight. Physicals required for sixth grade will fulfill the sports requirement. Download the physical form: IESA-PhysicalCard


A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage; and death if not recognized and managed properly. In accordance to the IESA policy and Illinois State Law, a student athlete who is suspected of sustaining a concussion of head injury in a practice or game shall be removed from participating in practice or competition at that time. We recognize and appreciate the importance of athletics for both boys and girls at I.C.S. We’ll do our best to strive for excellence in such a worthwhile program.

A list of students participating in sports will be compiled. That list will be compared with student achievement and conduct each Friday during sport’s seasons. Teachers are asked to single out students who are not completing work, not working to the best of their ability, or are uncooperative in their conduct. The child and parent will receive a warning on the first occasion that his/her name is listed.

He/She will receive only one warning per sport’s season. On the second occasion that the name is listed during the season, the student will be removed from the team and not allowed to attend the practices for a minimum of one week.

The purpose of these standards is to try to help the student. We feel that sports is a very beneficial activity and are very much in favor of student participation. Nevertheless, there are academic and conduct expectations that must be met before a student is allowed to participate in athletics. We expect your cooperation and support in these matters.

Physical exams are required for students participating in team sports at I.C.S.

Athletes are also subject to team rules made by coaches and by schools playing host to any of our athletic events.


Each student is assigned a locker at the beginning of the year. Locks are no longer to be used on the lockers. We ask that students not leave food in a locker overnight. Periodic inspections will be made.
